The Department of Pediatrics has an exciting opportunity for a full-time Sr. Case Worker. The Senior Case Worker serves as the primary development resource for families and delivers direct social services to families. This direct service position is responsible for providing maternal child health services to clients (pregnant women and infants) participating in the Healthy Start/CIR CONNECT Programs and related special projects. Duties also include quality assurance and quality improvement activities related to service delivery. Must be willing to conduct home visits with personal vehicle to provide services to clients and interact with other community providers. Position is salaried, and requires working evenings and weekends as needed to ensure that job duties are met.

  • Provide maternal child health services that may include: making initial contacts/phone calls, recruitment, and referral of eligible participants; conducting client assessments; developing individualized care plans to address service needs and risk factors; and providing comprehensive care coordination and wraparound services including parenting education, breastfeeding support, tobacco cessation, inter-conception care education, childbirth education, and psychosocial referral services as needed.
  • Assess client needs for housing and vocational training and make referrals as needed; establish partnerships with local housing providers and employers to further facilitate referrals.
  • For high risk clients with substance use or substance exposure risk-factors, develop a plan of care to address risks, assess motivation to engage in treatment, and make treatment referrals.
  • Monitors outcomes, data collection, and record keeping.
  • Leads weekly group supervision meetings for staff.
  • Meets monthly data and budgetary reporting requirements.
  • Participates in visits with physicians during scheduled office hours or by appointment.
  • Assesses developmental progress at home and office visits and promotes knowledge of development progress.
  • Maintains a resource and referral file to assist families in finding timely and appropriate community-based programs and resources.
  • Scores and interprets developmental assessments.
  • Maximizes the continuity of care provided to enhance developmental potential.
  • Maintains ongoing communications with family members to capitalize on service effectiveness.
  • Participates as a member of an interdisciplinary team while serving as the single point of contact for families in need of early intervention services.
  • Ensures the provision of early intervention services in a cost-efficient manner.
  • Contributes to the development of a family support plan and monitors its implementation and modification.
  • Coordinates activities with other case-related medical and non-medical personnel.
  • Tracks services provided to each family.
  • Adheres to University and unit-level policies and procedures and safeguards University assets.

MINIMUM QUALIFICATIONS:

  • Bachelor's Degree in relevant field required.
  • Minimum 3 years of relevant experience
  • Must be willing to conduct home visits with personal vehicle.
  • Skill in completing assignments accurately and with attention to detail.
  • Ability to analyze, organize and prioritize work under pressure while meeting deadlines.
  • Ability to process and handle confidential information with discretion.
  • Ability to work evenings, nights, and weekends as necessary.
