As an RN Care Manager, you'll work in our bustling Family Practice offices in Barrington and Lee, NH, collaborating with an interdisciplinary healthcare team. Your primary responsibility is to manage and coordinate patient-centered care for individuals with complex and chronic conditions, helping them achieve their health goals through personalized care plans and coaching. Other locations may arise as needed.
What You'll Do:
  • Patient-Centered Care: Conduct comprehensive health assessments and develop individualized care plans in partnership with patients, caregivers, and healthcare providers.
  • Proactive Care Coordination: Manage care transitions and collaborate with hospitals, specialists, and community resources to ensure seamless care.
  • Health Coaching: Empower patients to take control of their health through coaching, focusing on self-management and shared decision-making.
  • Collaborative Teamwork: Be a leader in organizing the care team around patient needs, working closely with an interdisciplinary team that includes providers, pharmacists, and other healthcare professionals to address challenges and drive improved patient outcomes.

Qualifications
What We're Looking For:
  • Bachelor's Degree in Nursing (or equivalent experience) and an active RN license in NH.
  • 3+ years of nursing experience in inpatient or outpatient settings. Care management experience is preferred.
  • Strong clinical assessment, health coaching, and patient education skills.
  • Proficiency in medical standards of care and clinical nursing skills.
  • Ability to work both independently and as part of a team.
  • Experience with health information technology and electronic medical records.
Preferred Qualifications:
  • Certification in Chronic Care Management is a plus!
