The new University of Iowa Health Care North Liberty Campus will open in the Spring of 2025. This 469,000 square foot facility will be home to UI Health Care's nationally recognized Orthopedics service line, offering comprehensive, coordinated, and accessible care across the outpatient, inpatient and surgical venues. These state-of-the-art clinical facilities will also include a full-service community imaging center, laboratory services, physical therapy, retail pharmacy, and emergency department as well as an array of support services. The facility has been designed to support a superior patient and staff experience; excellence across our patient care, teaching and research missions; convenient access to services; and engaging community partnerships. The University of Iowa Healthcare, Care Coordination Division is seeking a Nurse Clinician to function as a clinical nurse expert and care coordinator for the assigned patient population facilitating seamless transitions across the continuum of care. The nurse functions as an administrative and clinical liaison for individuals to optimize quality health care delivery using clinical assessment/screening tools, education reinforcement, follow-up on the plan of care recommendations and guiding the utilization of cost-effective resources. This position will manage a specified population and work closely with other care coordinators during transitions of care as well as other interdisciplinary team members across the Continuum of Care.

Duties to Include:

  • Collaborate with nursing staff and other health care professionals in the development and review of standards and in planning, implementing and evaluating patient care for a specific patient population. Communicate with physicians and members of the health care team directing patient care.
  • Provide direct nursing care, facilitate continuity of care and follow universal precautions. Coordinate and facilitate patient care between multiple disciplines including home health agencies, referring physicians and other community support services. Coordinate patient care to better utilize supportive services through patient visit and possible admission, pre-schedules testing, follows up with test results to both patient and physician, obtains medical history, coordinates follow up visit appointment, provides medication, rehabilitation teaching, etc.
  • Serve as a clinical resource or consultant for specific patient population. Participate in development and implementation of managed care programs for a specific patient population.
  • Provide leadership for direct patient care programs in area of specialization collaborating with unit-based nurse management team(s). Participate in the ongoing implementation of current regulatory standards in order to meet state and national agency accreditation.
  • Design and implement patient/family health education activities for the patient population. Serve as an educational and clinical resource to patient/family/staff. Support the implementation and evaluation of staff education and orientation programs. Facilitate the educational experience for health care students and monitor the care delivered.
  • Incorporate the role of patient care provider, advocate (patient, family, and health care professional), consultant, educator, researcher and quality manager.
  • Use relevant nursing research to improve clinical practice and investigate nursing care problems.

Percent of Time: 100%

Schedule: Monday - Friday, shifts primarily between 7:00 am and 6:00 pm. Some holiday coverage required.

Required Qualifications
  • A Baccalaureate degree in Nursing
  • Reasonable (1-3 years) nursing experience
  • Current license to practice nursing in Iowa
  • Demonstrated expertise in specific clinical area of practice
  • Excellent written and verbal communication skills.
  • Two years recent medical/surgical inpatient experience, including care coordination
  • Proficiency in computer applications including Microsoft Excel, Word and PowerPoint
  • Demonstrated excellent organizational skills, critical thinking, and problem-solving skills
  • Demonstrated expertise with patient/family discharge planning process
  • Experience working with an interdisciplinary team
Desirable Qualifications
  • Previous case management experience is desired
  • Certification in case management is desired
  • Previous experience with EPIC is desired
