Purpose

The CWS Fort Collins Reach Navigator - Health and Well Being, will be part of a team whose purpose is to welcome refugees, humanitarian parolees, special immigrant visa holders, and other Office of Refugee Resettlement (ORR) eligible newcomers to Colorado with supportive and bundled services that lead to early self-sufficiency and employment, social adjustment, educational and economic empowerment, and healthy living; to engage with refugee families to help them move from arrival to initial stabilization and self-sufficiency in an array of domains; and, to assist refugee families in attaining success in the first steps toward integration and set them up for success over the long-term.

The primary purpose of the Health and Well Being Navigator is to assist eligible participants, with a focus on an intentional linking of individuals and families to the healthcare system. Establishing a medical home is key to ensuring individuals and families receive preventive and ongoing care. In this spirit, the Colorado Refugee Services Program (CRSP) grants with Federally Qualified Health Centers and other local medical providers to facilitate immediate and ongoing connection to care. Reach health services must include services that support the completion of a medical screening and link refugees to healthcare and services that can support their well-being over time.

This is a hybrid position, based out of the CWS Fort Collins office with a minimum of 2 days in-office. The Navigator will work directly with incoming families, with the support of an interpreter as needed and will be required to complete a background check, confidentiality agreement, and training modules, treating all individuals with compassion, understanding, and respect.

Responsibilities

  • Conduct trauma-informed intake and screening;
  • Provide comprehensive family assessment (FSSP) using standardized instruments; Meeting with clients to gather information, fill out questionnaires, and assess their distinct needs for support services.
  • Check eligibility for refugee cash assistance, employment incentives, and other income supports and provide them in a timely manner
  • Provide supportive services to assist refugees in applying for and maintaining benefits for which they are eligible;
  • Regularly connect with local service providers, community-based organizations, and other agencies throughout the State of Colorado to ensure that eligible clients are provided the basic tools necessary to meet their established short-term goals and objectives for their resettlement in the U.S.
  • Assess and monitor employment progress at regular intervals to ensure progress on goals is made, designated resources are maximized and modify initial resettlement plans and other case file documents as required.
  • Provide Health related services for individuals receiving benefits; Health services include:
    • refer refugees to the appropriate refugee health screening provider within the timelines outlined in the Reach guidelines.
    • ensure transportation to all screening-related appointments, by arranging ride-share or facilitating public transportation use.
    • assists clients with completion of release of information or new patient registration forms specific to primary care location.
    • submits clients information to the Newcomer Health program via the state database, for data reporting purposes.
  • Maintain accuracy in client case files and case logs; update supervisor on case status; ensure compliance with case file management and reporting requirements, including timely data entry in databases.
  • Perform other duties as requested to support team needs and enhance program effectiveness.

Qualifications

Education:
  • BA required
Experience:
  • Minimum of 3 years of relevant experience including working in case management with refugees.
Other Skills:
  • The successful candidate must be highly detail-oriented and possess superior organizational and task management abilities.Respect for client confidentiality. Strong computer literacy with knowledge of word processing and database applications, and ability to use standard office equipment in performing varied activities. Superior written and verbal communication skills.
  • Commitment to diversity, equity, and inclusion and willingness to support CWS' Platform on Racial Justice as a CWS employee required.
  • Bilingual skills in English and a current refugee caseload language is preferred but not required (Spanish; Haitian-Creole; Ukrainian; Pashto; Dari; Tigrinya; Oromo; Arabic; Amharic; Kachin; Burmese).

Special Requirements

This is a Hybrid position with a minimum of two days in office requirement.
