The Liaison Specialist is responsible for the implementation of the goals and objectives within the care coordination model/s set forth by Compass Health and SSM Health. Participation goal in this care team model is to provide patients with an ongoing relationship with the team (including Morning Huddles) and continuity of care through all stages of care.

This is a Full time, benefit eligible position. The schedule is Monday - Friday from 8 am to 5 pm. This position is located at SSM Health Hospital in Fenton.

ESSENTIAL FUNCTIONS

* Provides timely and relevant reports to SSM St. Clare Manager, Case Management regarding status of the patients' goals and objectives established by the model/s
* Assists in developing and maintaining an objective, reliable and consistent database of information on program metrics and outcomes
* Participate in providing formal and informal education based on community needs
o This will include networking and the establishment of community support through the attendance of community relations meetings and professional alliances
* Minimizes errors and maintains high quality by checking or monitoring data in a timely manner and by developing and maintaining systems for organizing work and information effectively
* Provide referral coordination services such as dental, medical and behavioral health services as needed by the patient. Ensure that patients needing to see multiple clinicians and/or receive diagnostic tests will be coordinated in a way that bests suits the individual served needs
* Follow-up with all referred patients after a discharge from SSM St. Clare Hospital or ER visit and assist with scheduling for these clients as needed for follow up care and services
* Works in addition with other team members to identify and/or develop a referral system with community-based organizations, schools, employers/business, etc. and track the source and number of referrals. Make referrals to community agencies on behalf of patients, if applicable
* Provide care coordination services through: (a) Assessment of the individual/family needs and strengths and develop a plan for assistance, (b) To provide direction, support, guidance, education, and training for the recipient of services through Interventions, (c) Provide advocacy, information, referral, and follow-up services to verify the promotion of the welfare of the patient, and (d) Maintain current documentation according to the policies and procedures
* Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to
ensure a professional, responsible and courteous environment
* Promotes effective working relations and work effectively as part of a team to facilitate the Clinic's ability to
meet its goals and objectives
* Train and implement the PRAPARE assessment
* Promotes wellness by providing culturally appropriate health information and education to patients
* Encourage patients to make steps toward promoting their health and managing their diseases or conditions
* Advance patient's health literacy related to their condition
* Other duties as assigned



High School Graduate or GED required

* One (1) to Two (2) years of relevant work experience required
* Familiar with medical terminology, insurance coverage, and inpatient/outpatient hospital services
* Proficient in Microsoft word, excel, outlook preferred

LICENSURE/CERTIFICATION

* Successfully complete the accredited Community Health Worker program
