Compensation: $25.54/hr ; $26.81/hr (QMHP)

The Community Support Specialist Disease Management is responsible for conducting outreach and coordination of care for all active disease management clients.
ESSENTIAL FUNCTIONS - JOB SPECIFIC:
* Conducts in-person and phone outreach to all active DM clients monthly and will send an outreach letter to all active DM clients.
* Coordinates with BHL team on all DM clients who are incarcerated.
* Conducts outreach to DM clients who are currently hospitalized by phone and attempt to coordinate care with the hospital.
* Confirms client Medicaid status and recent hospital stays, ER visits, medications, contact information, and pharmacies.
* Coordinates with internal therapy, psychiatric, and health resource specialists to move clients to community support services.
* Works with the client to identify barriers to getting enrolled into services.
* Attends all integrated meetings in offices where CAPs are located to coordinate care as needed for active or inactive DM clients using the crisis centers/any crisis service.
* Documents all coordination of care and client interactions within 24 hours.
* Maintains a tracking system for all clients needing contact each month.
* Coordinates with HALs to obtain virtual assessment appts for the DM client of requested.
* Attends all ERE and HAL meetings and huddles to have full integration into each team.
* Collaborates monthly with DM coordinator on cyber/active DM client lists to ensure they are up to date.
* Inputs DM data into Care Manager as needed.
* Completes monthly chart reviews on cyber monitoring clients
* High School/GED required
* Associate's/Bachelor's degree in a human service-related field preferred
* At least four (4) years of experience OR a combination of higher education and experience required if a High School/GED is the highest education obtained
* At least two (2) years of experience OR a combination of higher education and experience required if an Associate's degree is the highest education obtained
(*If the degree is an Associate's of Applied Science in Behavioral Health Support from an approved institution; then no additional experience is required)
* At least one (1) year of experience required if a non-human service Bachelor's degree is the highest education obtained
* All experience must include working with individuals with psychiatric, substance use, and/or developmental disabilities
* A valid driver's license and agency established minimum automobile coverage required
