Sandra Eskenazi Mental Health Center, Indiana's first community mental health center, provides comprehensive care for emotional and behavioral problems, including severe mental illness and substance abuse. The Sandra Eskenazi Mental Health Centeroffers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

Job Role Summary

The Care Coordinator Pathway Intern learns under the supervision of the Field Instructor, and in collaboration with the treatment team, to serve clients by conducting needs assessments, providing skills training, assisting clients in formulating treatment goals and collaborating with clients to revise person-centered treatment plans. The Care Coordinator Intern advocates for clients to ensure access to services which include developing community relationships and obtaining needed resources. The Care Coordinator Intern participates in all learning platforms to develop knowledge, skills, and abilities to function effectively in the Care Coordinator role.

Essential Functions and Responsibilities

  • Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of Motivational Interviewing
  • Learns and practices conducting needs assessments
  • Learns and practices collaborating with clients on treatment goals and revising person-centered treatment plans
  • Learns and practices providing skills training and development to clients in group and individual settings as outlined in the person-centered treatment plan
  • Learns and practices linking clients to needed resources and community supports including working with family and significant others to meet treatment plan goals.
  • Provides crisis management and emergency interventions including but not limited to cardiopulmonary resuscitation and approved area-specific crisis intervention techniques
  • Reports to Field Instructor and treatment team in staffing including but not limited to successes and challenges, assessment of client progress, barriers to success, and any level of care changes
  • Learns and practices providing the appropriate service at the right time, right place, and right frequency as determined by the client's level of need and treatment plan goals
  • Learns and practices meeting the agency's standard of timely completion of all documentation including ensuring progress notes meet guidelines for timeliness and accuracy
  • Learns and practices maintaining a safe environment at all times through on-going assessment of milieu/appointment environment and intervenes as necessary or as directed by Field Instructor or program leadership
  • Meets agency requirements for continuing education

Job Requirements

  • Enrolled in Senior year of CSWE certified Bachelor of Social Work Program
  • Certifications in Cardiopulmonary Resuscitation
  • Valid Indiana driver's license preferred

Knowledge, Skills & Abilities

Knowledge
  • Basic knowledge of the tenants of Social Work and the Social Work Code of Ethics
  • Basic knowledge of substance use disorder and/or co-occurring disorders
  • Knowledge of client rights and responsibilities
  • Must meet all population-specific competencies for role and service area
Skills
  • Basic computer skills
  • Strong interpersonal skills
Abilities
  • Ability to learn how to teach a skill based upon person-centered learning needs and learning styles
  • Ability to be both clear and concise in written and verbal communications
  • Ability to work cooperatively in a multidisciplinary team

  • Ability to provide hands-on client care
  • Ability to perform safety interventions including physical safety interventions required for service area
  • Ability to lift a minimum of 50 lbs
