Job Summary:
As a Nurse Practitioner, you'll conduct in-home wellness visits and risk assessments for Medicare members. Your role involves comprehensive health evaluations, patient education, and improving care quality.
Responsibilities:
- In-home assessments
- Physical exams and medication reviews
- Patient education
- Relationship building
- Quality improvement
- Valid NP license
- In-home assessment experience (preferred)
- 3 years of patient care experience
- Strong communication skills
- Reliable transportation
- EMR proficiency
- Competitive pay 1099
